Agisce a favore di un target di persone vulnerabili anche in collaborazione con altri enti e servizi. Favorisce l’accesso al SSN a chi ne ha diritto. Promuove iniziative culturali e informative a favore della popolazione vulnerabile, con l’obiettivo di diffondere conoscenze e prevenire e abbattere stigma e pregiudizi in un’ottica di solidarietà. Le attività infermieristiche vogliono dare una risposta ai bisogni che vengono identificati e pianificati come ad esempio:
• Medicazioni semplici
• Valutazione e aiuto per la corretta assunzione di farmaci
• Rilevazione di parametri vitali e antropometrici
• Valutazione dello stato dei piedi in particolare per soggetti diabetici
• Aiuto nell’identificare la rete supportiva in relazione al bisogno identificato
